Belt keeps coming off.....

Hi All,
Got a problem. When I was pushing in the clutch petal, it didn't want to do all the way down, acted like something was holding it. Finally it did and the belt came off. I put the belt back on and pushed in the clutch again. This time the belt was really lose and it came off again.
What could be the problem?

Do you have the belt on all the pullys?

Thanks for getting back to me. I think they are on all the pulleys. I have been trying to find a diagram of how the belt goes. Can't seem to find anything on the drive belt.
